Submitting a Work Order (making a general request)
You may submit a work order to the OTS ServiceDesk using our electronic request form, e-mail or simply dial 7385 from any phone on campus and an OTS representative will take your request. (off campus- 936.273.7385).
To submit a work order you will need the following information:
- Nature of the problem (what is wrong?)
- Client type (student, staff, faculty, etc.)
- Room (number) where the problem is located (lab,classroom, office, etc.)
- Request type (what you would like done)
- Item(s) affected (hardware, software, network, etc.)

Upon completion of the Work Order you will be notified by e-mail.

Submitting a request for Audio/Visual equipment. To submit an AV request, you will need the following information:
- Date Needed/Finished (xx/xx/xxxx)
- Time Needed/Finished (xx:xx AM/PM)
- Equipment Needed (include any accessories you may need)
- Building/Room where equipment will be used

Using our electronic forms
Our electronic forms are just as simple and reliable as calling the ServiceDesk.
When filling out a form, you do not have to enter data in every field (when a field is required you will be notified).
However entering more data will create more precise results.
