Faculty Guidelines for Testing Center

1. Instructors must complete an Examination Request Form specifying: the students permitted to take the exam, the exam guidelines, the materials allowed, the time allowed, the last date the test may be administered, and any special services that may be needed. Instructor contact information is required on the form. An electronic online form (for e-mail) is available on the Lone Star College–Montgomery - Lone Star College–Conroe Center web site in the Information for Instructors section. 2. Online tests requests must be submitted online.(e-campus, WebCT, etc.) Passwords are required for all on-line exams that are expected to be proctored in the Assessment Centers. Exams without passwords will not be administered or proctored by the Assessment Centers and the student(s) will instead be encouraged to utilize the equipment and assistance provided by each campus's open computer labs.


3. Rosters are required from all new instructors.


4. All tests must have a specific subject or title to refer to the specific class information for the student and the student's last name.


5. New instructors dropping off or picking up tests, must show an ID if requested.


6. Instructors must log in all tests in the Testing Center and pick up completed tests within 2 weeks.


7. Software needs will be addressed by each campus Assessment Center through a completed on-line Software Request Form. The form is available on the Lone Star College–Montgomery - Lone Star College–Conroe Center web site in the Information for Instructors section. Instructors are encouraged to submit this form immediately following the adoption of textbooks prior to the beginning of each semester. NOTE: Software needs is the responsibility of the faculty member's division. The form is available at (montgomery.lonestar.edu/11524).


8. Please allow students at least 3 - 7 days to complete the exam. The end date of the exam must not fall on a Saturday or Sunday.


9. The Assessment Center will not accept or distribute any non-testing materials, such as syllabi, projects, take-home tests, or homework.


10. If an exam is e-mailed to the testing center, we will print no more than 3 copies.


11. No faxed tests will be accepted.


12. Please notify the Assessment Center of students who will need any special accommodations or special arrangements.


13. The Assessment Center will provide the instructor with an incident report stating any problems or special concerns (cheating), that may occur during testing. All materials will be confiscated and the incident report will be sent to the instructor.


14. Important: Please tell your students they must have a student ID or photo ID to take any test.
